AI-supported treatment for patients with chronic illnesses
As Denmark experiences increased lifespans and improved wealth, chronic conditions such as diabetes, heart failure, and chronic obstructive pulmonary disease (COPD) are becoming more prevalent. Managing these chronic conditions presents significant challenges, particularly in preventing complications and minimising hospitalisations.

Traditional healthcare models often react to worsening symptoms rather than proactively managing patient health, leading to increased strain on healthcare resources and diminished quality of life for patients.
Aim
The project aims to transform the care of chronic patients by shifting from a reactive to a proactive model by integrating advanced artificial intelligence (AI) into an existing telemedicine programme. The project enables the early prediction of exacerbations and complications. This proactive approach facilitates timely interventions, reduces hospital admissions, and enhances overall patient well-being. Ultimately, the project seeks to improve patient outcomes and optimise the use of healthcare resources through early, targeted care.
Partners
The project is a collaboration between Appinux, Copenhagen Data, Middelfart Municipality and CAI-X at Odense University Hospital.
Funding
The project is funded by the European Regional Development Fund, the Decentralised Business Development funds, and co-financing from the project’s partners.
